If I call out a stranger on the street as "hey, you, the obese one, can you move?" It is perfectly reasonable to read that as some sort of insult or slur.

Yeah, it is not about mixing up "facts" and "feelings". Sometimes people need to justify their lack of empathy on the technical status of a word.
