It varies by country and age, of course, but usually between 0.8-1.5%. This is for specifically life-threatening things.

These are basically WHO/CDC definitions. Of course the author could use whatever definition she wants, but it's not clear whether she's lumping in all 'serious' complications with 'life threatening ones'.

Yes, or a similar error in a source somewhere. Even pre-industrially, mortality rates were ~1% per pregnancy, which is about what you expect with ~1.5% untreated complications.
