Look at the VA. They have coverage but often go without medical care for a variety of reasons.

True. After all government healthcare changes last 10 years my Mom has to wait 3 months to follow up with her GI doc, due to enormous amount of patients. Any issues go to ER, but ER does not handle her non-life threatening issues, so has to wait for GI doc to get freed up later
