    The role of Cardiology is to risk stratify and modify preoperatively. It is NOT to determine surgical candidacy (“clearance”). This is determined by the patient, the Surgeon, and Anesthesia.

    Can we give a shoutout to the employees who are or environmental service workers? They save lives and prevent disease by thoroughly cleaning and disinfecting hospital rooms over and over, especially on really busy days. I work with great ones!

    50M with heavy alcohol use and cardiomyopathy has LVIDd 7.0 cm and EF 20%. VT and ICD placed. Stops alcohol and titrated to max-tolerated GDMT --> one year later, LVIDd 5.0 cm and EF 45%. 8 years later, ICD at end-of-life. NYHA I. EF 45%. Do you recommend ICD replacement?

    Warning: ahead. . . The other day, I convinced 2 pts they didn't need requested stress tests: 1) exercising 1h 3x/week w/o limitations = passing your own stress test 2) a normal stress test is no guarantee against "the big one"- control of risk factors w/meds is.

    99% agree. Just please don’t do it in front of the patient. Pull me aside, give me your thoughts, and we can modify the plan.

    I do not mind if a nurse questions my actions at work. I am not always right. Sometimes I think I am, and yet I am wrong. I don't want to fall into the black hole of cognitive dissonance. Question me. Keep me on my toes.

    Sometimes, when the patient speaks, their spouse/partner rolls their eyes or sighs or looks annoyed. Try to figure out why – there’s usually an important story there. Pay attention to dynamics- you’ll learn a lot!

    What I’ve learned thus far: 1. originated in China but is not Chinese. The virus does not follow racial transmission. 2. is contagious but is not as fatal or deadly as many would have expected. 3. is not a reason to divide communities.
