In theory, with the encroachment rule - if the goal is scored and the attack team were in the box = retaken. If missed, not. And vice versa for a miss and defending player encroaching on a pen. So I assume that's how it is handled ...
Is stepping off your line a foul only if the penalty is saved? ie could you keep stepping off your line in order for a goal never to be scored?

It's always a foul, it just gets dealt with differently if the goal is scored or not. Basically, it's always a rekick, unless the goal is scored, in which case it stands. The real issue is that keepers come off their line all the time, and it's rarely gets penalised.
