Actually, it's the norm for transgender patients to know far more about transgender healthcare than most medical practitioners. We're the ones who have to educate them; not the other way around.

Sex isn't 'assigned' so much as it is observed. It's assigned (usually w/ needless surgery) only in the case of the few intersex people who have ambiguous genitals, usually with tragic results. Gender is the box we're placed into. We're sliced up to fit those roles.

Also: A mental illness/disorder is a clinically significant thing causing sig distress + effects on several areas of life, according to the DSM-5. Dysphoria therefore is one. If trans doesn't include dysphoria, it's not, but that makes no sense.
