Scenario 1: man is caught in a terrible fire, is permanently scarred so as to be totally unrecognisable but is unaffected psychologically. Ask his wife "is this the same man as before the fire?" and she would say "of course he is. He's the same man - he just looks different.

Interesting existential question: I'd say the brain, as it's affected by/responds to physical/emotional stimuli, hence thoughts, feelings, etc. But to what extent do we factor in those stimuli? (And no, I don't think this is relevant if we're talking about biological sex...)

Hmm... No brain exists independently of other stimulus though (except for those of a few, probably very unfortunate, recently deceased pigs).
