Growing up in a Cuban household I learned attorneys were always addressed as “doctor,” but only during legal consultations or official functions where their line of work was the focus. Otherwise, in all other instances you addressed them by their names.

Entirely sensible! A Ph.D should be called "Doctor" at an academic conference.
