It's not about separating the biological and the psychological factors. It's about recognizing that most of the harm to users comes from the way society treats these drugs.
Oh definitely, there’s no doubt about that. The opioid epidemic reminds me a lot of AIDS and crack epidemic in the 80s. How Reagan responded was horrible. The social stigma compounds a persons suffering.

But, we generally treat people with any form of illness pretty appallingly. Yes, the social stigma around drugs & addiction pushes people underground and makes people are afraid to seek help. We stupidly still treat addiction as a weakness and personal failing.
