Every department has abusive labs that many grad students leave. My question: what accountability measures are in place in your dept to address these issues from the student or faculty side?

One key component is to have a safe-channel for students to report abusive event. Truth is students usually believe faculty are not on students' side. Having a PhD program office independent from the Dept. will help a lot (like in many umbrella programs).
Most students are aware complaining is the worst career move you can make. Also the Faculty are not on the students side. In my experience complaint procedures just exist to silence victims while simultaneously making it appear like there is transparency and accountability.
