Article says they were sent an email with 2 options: endorse Sinema or stay neutral. Why couldn't they get a third option to endorse Mcsally?

Just because you don't actively pander to unions doesn't mean you don't like them. Generally it isn't private sector unions that bother folks, it's public sector ones. They endorse Democrats to lobby on their behalves while taxpayers foot the bill.

Public sector workers earn their pay just like everyone else. The tax payers are paying for the services they provide.

The current Republican Party is trying (and succeeding) to systematically dismantle unions and worker protections in general. So any union that supports a Republican is suicidal.
