Not that it makes it OK, but it appears that photo was sent with AP wire, and those sites autopublished it.

I hear that—but I imagine a diverse staff that reflects the U.S. would have caught it right away before pubbing...

This is actually my issue with autopublishing wire like this, you get blamed for mistakes you would have caught otherwise.

I do think AP carries a lot of blame here—but it's not just AP that has to work to make it better 1/2

Even if they autopubbed, some of these outlets also manually tweeted the story, so people's eyes and hands were on it 2/2

Again, I'm not making excuses for anyone, newsroom diversity is a HUGE issue, but just trying to show the larger picture here.
