When I greet new people, I systematically have to think about what to say. Hello, your name, nice to meet you.. and sometimes I forget one of them.. how do people do this so naturally

I never really understood when people greet with a "what's up?" or "you alright?" (very UK thing) Am I supposed to answer the question, or am I just supposed to repeat what they said as a "greeting"?? I rather just start with hi
