Yep, basically scam insurers who covered very little and still would try to wriggle out of what they did.

I used to take calls from people all the time whose insurance coverage would cover screenings but no confirmatory testing

Or it would cover treatment but didn't include follow up blood work

I had physical therapy every week for a few months. Every time, they denied the claim, and I had to call and argue to get them to pay.
