I see the gender point. In this case, it's likely as much a matter of sex. If a trans girl had gynecomastia, she would be eligible for getting the surgery/treatment as well.

Isn't some of the trans advocacy asking for insurance coverage, not only informed consent ( as with many cosmetic procedures)? E.g., insurance for breast reduction for females/girls depends requires medical approval, a record of complaints, never just "informed consent."
