‘Obese’ has a ton of use as a clinical term in the mainstream though. ‘Narcissist’ is not and has never been a stealth insult in any imaginable way. It’s purely an insult in every context outside the DSM5

Even before what I'd call the "narcisphere" (what Kristen Dombeck wrote about in The Selfishness of Others) really became a Thing in the Discourse, throwing around "narcissist" was a pretty big cultural problem

It is very, very easy to perform Russell conjugation on the concept of narcissism -- "I'm self-confident, you're self-absorbed; I'm self-aware, you're self-obsessed; I stand up for myself like a man, you put yourself ahead of everyone else like an asshole"

The correct and proper amount of self-love a person should express and the judgment over whether that expression is appropriate and healthy and proportionate is wildly subjective and prone to extreme bias
