I know this is only going to bother me but the Second Lady is called the Second Lady because there’s a First Lady (the wife of the president.) if there’s no First Gentleman it shouldn’t need to be a Second Gentleman.

Well, no, it means Joe Biden himself is the First Gentleman (along with being the President), right? Technically "First Lady" is about the "order of precedence" thing they do when seating people at formal events, where if you have no rank yourself you inherit your spouse's

If the First Lady held an elected or appointed office in her own right you would call her that instead (Senator, Congressman, Director, etc) The "First Lady" thing is a placeholder title, the indication she holds the social rank without a corresponding office

I wanna say philosophically this is about having multiple titles that override each other The Queen of England is also a Duchess and a Princess and a Dame and so on but you name her by the highest title she has out of politeness
