I know this is only going to bother me but the Second Lady is called the Second Lady because there’s a First Lady (the wife of the president.) if there’s no First Gentleman it shouldn’t need to be a Second Gentleman.

Yes but as the president HAS an office, he doesn’t get the placeholder title, therefore there is still no first gentleman

I wanna say philosophically this is about having multiple titles that override each other The Queen of England is also a Duchess and a Princess and a Dame and so on but you name her by the highest title she has out of politeness

To be fair, First Lady doesn't actually mean the President's wife, it means the person who serves as the hostess for presidential events Someone else can do that job and be the highest ranking woman in the room, but usually only if the wife is unable to do it (or doesn't exist)
