So that's when he started his courtroom consulting company and how he met Oprah She knowingly presented him as a practicing clinician when he's literally not allowed to do that, everyone who goes to him for advice has to sign a waiver saying they know he's not a real doctor

It means both that he got his doctorate in a field that has nothing to do with infectious disease and that he's demonstrated to the authorities multiple times his ethics are not to be trusted

Because he sucks, and because a clinical PhD is still fundamentally different than an MD (an MD in behavioral health is called a psychiatrist not a psychologist)

He does not have "medical knowledge", a clinical psychologist is not a medical doctor and is explicitly not allowed to say he has "medical" knowledge, and yes this is a big deal
