People use ellipsis in a variety of ways colloquially. I understand not liking her insistence to be called a dr. in the public eye because of this common misconception, but it doesn’t mean she’s wrong

She’s not wrong & everyone knows that people with a PhD are doctors, I think the point is that even a lot of people (with/without PhDs) agree that if you’re not a medical doctor & force people to call you doctor & emphasize it at every given moment, you’re kind of arrogant.

It ought, then, to be equally "kind of arrogant" to insist being called "doctor" if you're also a medical doctor.

I would agree that unless you’re in a setting where it’s important, that would also be arrogant!

Yup, and the general public reasonably assumes that a person insisting on being called Dr. is a medical doctor, since Medicine is relevant to most people, whereas Academia is not.

Given that we all had to go to school and many of us have kids who are currently in school, education is certainly as relevant to daily life as medicine
