Not necessarily: “I’m smuggling in generic medicines for people who can’t afford the corporate price gouging!” vs. “Some of these smuggled medicines lack proper quality control compared to the official ones!"

Another option is smuggling a luxury good that has a high tax on it, but isn’t drugs. Historically this included things like butter and tea.

In fact the classic “Pirate Accent” is a Cornish accent probably because pirate stories built on an existing set of tropes from smuggling stories.

That certainly seems to be part of it, but The Pirates of Penzance has Cornish pirates and is all the way back in 1879.
