Ford is a researcher for the book, for which he spends 15 years on earth only to submit the entry "mostly harmless". I'd say you're pretty spot on there ;)

Well, admittedly, that's not completely his fault. It's brought up a few times that he wrote a really long article that went into a lot of (apparently not very accurate) detail. But it was the editors who first decided that "harmless" was the article and added the "mostly" later.

No, I think it's mentioned in the first book, elaborated on in a later book, and the last one Adams wrote had the full text of Ford's article show up.

Yes, in the first book/radio play Arthur demands to see the entry for Earth in the Guide because now it's all that remains of Earth in the universe and it turns out to just be the one word

Then we get the callback in So Long and Thanks for All the Fish where, because we're now in the brightest and best possible timeline, all of Ford's prose about Earth is suddenly restored, because Earth once again exists and is open for interstellar tourism

The final book, which is actually titled Mostly Harmless, delves into this business with multiple timelines and establishes that which Guide entry for Earth is active is a signal of which alternate version of Earth currently exists

It changes back to "Mostly harmless" because Earth exists again but it's gone back to a pre-contact version of Earth no one cares about (the one with Tricia McMillen the news anchor), not the one the dolphins restored
