3) I absolutely do hold Uber responsible and so did CS Unfortunately there's no way to hold them responsible without drawing attention to the individual driver because 100% of the evidence for the interaction involves the driver

That said, the primary way Uber is responsible for this specific situation is not vetting or screening or overseeing their drivers If they were a better company, this guy wouldn't have a job with them in the first place
