There's a Gavin Lyall thriller about shipping. The hero discovers the ship's engineer was drunk when the ship went down. Aha! Motive! The owner was hiding that so as not to invalidate insurance! ....

The insurance expert is like LOL, no, your crew being drunk and screwing up is the *main* thing you're insuring against, that doesn't invalidate anything.

Hard to believe but there was a time when insurance adjusters were only slightly less prominent than detectives as thriller protagonists.

In the end it turns out the engineer wasn't just drunk, he was in a drunk tank in Tallinn or somewhere. Sailing without a full crew breached the insurance conditions.
