But that whole "death to blasphemers/apostates" thing has a way of stifling any real reform.

this is a really important question. I think it could have worked. Two things screwed it:

flourishing of Islamic terrorism gave them a symbol to orient towards, and ambivalence to GWOT strengthened it

then sheer numbers in places like here gave them opportunity to isolate themselves from host culture
