I think the situation here is that building regs are quite clear and widely understood. Some senior people must have been aware things were being done wrong. I'll be shocked if no-one at any stage raised concerns.

The inspection regime responsible for enforcing those standards was routinely approving substandard cladding.

Most probably that too. Funnily enough my father is currently arguing with his local council about work they signed off on a new build he bought.
