Unpopular opinion: Permanently ban anyone who isn’t nice. Toxic players generate less revenue than they prevent by keeping out the people they harass. Give one warning and then kick them out forever. The thing is this can easily be abused too. Women will find themselves being instantly reported (& thus banned) by toxic players for no reason, simply for being women. (ie all the same reasons they get harassed in game.) These guys will exploit the report system to harass women.

Agreed -- any game truly interested in this would have to invest in a strong moderation team to prevent this, and player reports need to be manually vetted by that team. Ultimately, companies need to be interested in reform less than recruitment, whatever form that takes.
