If tutors really do reliably produce two-sigma learning gains, then why aren't most top achievers the product of tutors? Yes, they're expensive, but perhaps "only" 2-3x private school; there are a lot of wealthy parents. Diminishing returns for high achievers?

partly because all of us, including me, forgot that the tutoring portion only accounts for one of the Sigmas

Maybe! It's hard to imagine a serious tutor (i.e. outside the pressures of a state-mandated scope+sequence) not implementing something like mastery learning.
