I’m getting minor surgery tomorrow. And the only surgeon that was referred by my in-network doctor, is a surgeon not in my network. I have to pay. Why would this be?

Furthermore, I was quoted one price by the doctor and then two weeks later, I was quoted a different, much higher price. When I mentioned this, I was told they would honor the first price. How is it that a cost for surgery can fluctuate in a two-week period?
