The 96th St station of NYC's SAS Phase I was built using cut-and-cover. Same for all Phase II stations except the tiny part of 125 St that passes under the existing 4/5/6 tunnel.

It might be worth noting that in the 1930s the Feds were paying for much of the IND construction which let planners go wild with large mezzanines and flying junctions. Same thing happened when the Feds started paying for highways.
