Comment on the original: "We have had patients ask, “You are not going to tweet about this are you?” I think that would be so cool. If it were me, I'd want it to get boatloads of comments.

If you want your twitter feed to be your professional profile, keep it professional: no politics, be careful about family stuff, be careful about comments about non-professional topics.

I would add: When reading the tweets of others, don't assume that their twitter feed accurately reflects their work life. My twitter feed reflects my hobbies, rarely what I actually do.
