His punishment for trying to cheat death is to never have access to the peace of death. He gains immortality and the infinite cycle of chasing goal after goal after goal. It's an ironic punishment b/c it gives him what he desires.

But on a more serious note. I think that reflects inhuman design more than it does reflect systematic failures E.g. all elevators have mirrors because 1. Makes it look more spacious which reduces claustrophobia. 2. Gives people something to do on the way up
