They never saw something through end to end and were only concerned with making their section look the best. Or they have very shallow and segmented view of the world and ignore all the externalities.

Complex technology is a pandoras box of higher order effects. The worst PMs have a complexity fetish from a desire for more and more because they never see the total effects of complexity on the world.
