Placebos don’t always have to inert.

Agree & there is a note addressing it. But as documented by the RIAT researchers in some trials, subjects appear to be told they are getting an "inert, inactive" placebo when they're not.

"...43% of people taking placebo [experienced gastrointestinal side effects]. The study does not describe what was in the placebo pill, although, with such a high rate of side effects, it was unlikely to have been “inert.”" Speculation. Nobody had gas in a trial? Baseline rate?

A studies placebo need to be clearly defined in their write-up by their ingredients and administration, additionally placebo groups recipients also need to specify participants had other medical interventions or previously been used in other trials.
