I read that as saying "the functions in ctype.h or wctype.h beginning with 'is' or 'to", not "all functions beginning to 'is' or 'to'"

Unfortunately not. The intent is to make it possible for any identifier with those prefixes to be added to the corresponding header in the future. The spec itself (http://www.open-std.org/jtc1/sc22/wg14/www/docs/n2346.pdf …) is clearer (sections 7.1.3 and 7.31)
