Per Dr. Az, initially almost always there is a sex congruence. It's denied by patients seeking medical treatments; and some may have been too young to remember. /2pic.twitter.com/sczYGBkTqo

Then there is a vague feeling that develops that "something is not right/change in mood". Adolescence, anyone? /3pic.twitter.com/3SvfVciPQB
A period of search for meaning of these feelings follows. This is where the social media sites come in really handy @lacroicsz. And yes, our schools are very helpful with preemptively teaching that feeling "different" usually means you are born in the wrong body. /4pic.twitter.com/3jBoreDjuc
Eventually, the mind locks in on the perfect explanation: "I feel 'off' because I am born in the wrong body. Schools, doctors, even some parents validate it. What follows is increased obsession with gender, and beginning to think about your past through the lens of "trans." /5pic.twitter.com/92HXlT2ReM
As the mind goes into the obsessive loop of analyzing each thought and behavior through the "gendered" lens, it becomes more and more convinced of its own sex "incongruence," and the conviction of being "trans" is furthe reinforced. /6pic.twitter.com/8RwsRCsqbx
Dr Az is quick to point out that not every case perfectly matches this outline (but the vast majority do). The fact that this model was developed for the "traditional" GD, but has so much face validity when it comes to ROGD, gives us a lot to think about. /7
The most exciting part of the book to me is not just the model, but the solution. Psychotherapy to explore these factors appears to have a healing effect of GD. Majority of patients treated based on this model no longer seek surgeries or hormones, and regain piece in their body/8
This book will not uniformly please GC people. Many chapters have been written by the proponents of the surgical/hormonal route. Some will find the book giving too much to the pro-trans point of view. The lack of the discussion of ROGD-type onset is also a disappointment ,yet../9
The proof is in the pudding. Using Dr. Az's non-judgmental, neutrally framed, exploratory approach, the vast majority of GD sufferers resolve their conflict and do not need to subject themselves to medical interventions (which are still available as last resort). /10
