Exactly, you put it perfectly. They are completely blind to the damage they do.
They’re not blind. They know. They either don’t care (as nonbelievers are irrelevant) or they rejoice in it. Those that do care are those that allow their humanity to override their religiosity.
I think a third option is possible; that they dont believe what they do -is- damage.
Like @MurtadMilli who thought it was 'right' to wish someone dead, rather than being gay.
A twisted sense of righteousness, I think a lot of them suffer from such a delusion.
