okay I've done some preliminary reading on this, but I'm curious why no one has started a new Kaiser Permanente from the ground up instead of trying to acquire into it, or tack it on to their existing model

I don't think this is true. Do you have any examples? IMO IDNs are way less attractive and have low margins vs clear fee for service models that PE likes.
