I applied online for SSDI. The language is terrifying saying your commiting perjury if something is false. Part of my illness is cognitive issues so it took me weeks to fill out the online forms, have my husband check them and submit them.

I had to provide info for all of the doctors I have seen. I had to provide the dates I saw them, their address, phone number, fax number, all of that. Very hard for someone with my disability

I had a phone call with SS which also started out with that ominous threat of perjury. He asked me questions and told me he would send me "a form" fore to send back about my work history. Nothing else. No phone number to call back other than an 800# and only his last name given.

I get the "form" in the mail and it's probably 25 pages long and I am so overwhelmed I just ignore it for 3 days. I get my husband to help me and I still mess up on the form. I gather tons of pages of pay stubs, timesheets etc

All this to show that I stopped working full time in July. I tried to work in spurts after that but was never able to work more than 4 hrs at a time or more than 15 hrs in a week. With those maxes that was all I could do and spent the rest of my week recovering

I printed pay stubs from July to now showing my short term disability payments and hours worked (0 for most) and filled out the forms with all the money I received from STD, vacation, sick pay

I mailed it on 2/10 and got a letter a week later on 2/17 (dated for 2/12) saying that they haven't received my work form or pay stubs and if I don't send it by 2/26 I will be denied. THAT IS IN ONE WEEK

Also in the letter it says, "you must send us original records. If you don't have the original you must send a copy certified by the person who is the custodian of the original record." I don't have orijbal pay stubs it's all electronic. And I wasn't told this on the phone.

I called to ask if a print out pdf is ok and they said "that should be ok I don't know." They said the mail is probably here but hasn't been processed yet and to call back Friday. So if it isn't ok I have 3 days to get certified copies? I don't even know who the custodian is.

So then I said this letter also says you need a W-2 but he didn't tell me that on the call, is it ok if I mail that today and it's postmarked before 2/26 even if you don't receive it by then? She told me to drop it off. I said, I'm applying for disability. I can't just drivethere

If I call Friday and they tell me the printed pay stubs are no good I'm going to lose it. So I get 3 business days to get them to you but you get 120 days to give me a decision? When really you're just trying to find ways to deny me? I see what this is.

Sorry for the typos. My hands don't work also I'm mad. The work form should have said return this with original copies of pay stubs and a W-2. It said none of that and now I get notice that I have a week to get it in and they can't even tell me if what I sent in is ok.

Another note about how purposefully hard they make it. The letter says, for your convenience, return these items on the enclosed envelope. THERE IS NO ENCLOSED ENVELOPE! mayhem. Mayhem all around.
