Pretty much instead of a " public option" we are slowly being forced in a " private option. Basically means all the available options are consolidating similar to the airlines. This will cause prices to sky rocket and quality to fall.

All the focus on manufacturers and everyone forgot to watch the consolidation on the payer / dispensing side. Humana only had $55B in revenue last year. So after this we will have a choice of 3 pharmacies , 3 wholesalers , 3 PBM’s and 3 health insurers . can’t foresee any issue

Just today I had to convert a patient stable on suboxone for >6 months to zubsolv because humana changed the formulation of buprenorphine that was covered; expanding their reach seems unhelpful

BUNAVAIL (like subsolv) recently approved also has better bio availability than suboxone so you expose yourself to less drug (I'm not sure if it comes in 0.Xmg or mcg microdoses like belbuca & butrans patch for maintenance taper though)

But remember every person is different. Some people do horrible with conversions. Hate when patients are so stable and become unstable due to switching meds due to insurance issues

After 6 months I would hope the MAT doc Got the oud to the lowest dose possible suboxone taper. Long term high dose exposure leads to symptoms docs throw benzos adderol & antidepressants at. Without highest bioavailability w lowest dose possible you are headed for a train wreck.
