You or someone else can raise the money on his behalf through gofundme..& just provide the money to him in cash or pay the rent for him directly. We are definitely not a country which takes care of our most vulnerable and I hope leaders like you will be able to help change that.
You don't need to use his full name, I don't think. Then post the link so we can donate to him. In the meantime, are there any agencies that can help? Check blue pages of phone book, or go to the city website. Look for senior services or any emergency srvcs. DM me for more ideas.
