That form is not for signing up. That form is just for expressing interest. There was a survey sent out to TwitchCon ticket holders, asking if they wanted to compete in Twitch Rivals. Invites were sent based on those responses.
Even if we did fill out that form and already owned a ticket, that didn't guarantee an invite so far though, correct? Making sure we did everything correct as my team and I did both those things over 2 weeks ago

Correct. There are a limited number of slots, so invites have been going out in waves each day to those who already purchased tickets & expressed interest in competing in Twitch Rivals. Invites will continue to go out through the weekend.
