Usually one of 3 ways: 1. Companies report issues that they've found through their own safety testing and/or customer complaints. 2. Regular ppl report product safety issues to CPSC or the news media and we conduct an investigation. 3. Our own independent testing of a product

Occasionally, third-party groups (state AG offices or other safety-minded organizations working with a private lab) will send potential safety issues to CPSC as well. We can't comment on these investigations since they're confidential, but generally one of these ways.
