The point is, the power coming out of a cord that is plugged in through a USB adapter isn't that high

Unless there was a bridge between mains & the LV aside of the transformer.

If mains voltage somehow leaked onto the output end of the charger, it could easily kill someone. This is part of why GFCI is so important.

It’s also why it’s generally not recommended to buy cheap electronics that don’t go through safety testing.
