All this takes to happen is a improperly placed piece of shoring, not saying all the engineering is sound but this is what I do for a living and we all rely on the carpenters who build it.

We have a sign off sheet where we do a full walk down of formwork with a site engineer checking against design drawings. No concrete ordered until full sign off even on the high speed/ high pressure jobs

Yes definitely most/all sites have this. But the way everyone is pushed to meet or beat ( rarely happens) deadlines often leads to mistakes. It's easy to say what is wrong, right or production needs a lift from a desk. I'm about my safety first and the others around me.

I’m off work because of a site accident and Ive been totally safety conscious. Accidents happen even when we do our best to prevent them.

Not scaffolding! That’s a deck collapse during a concrete pour. That’s a third world issue and has no place in a modern construction world

Disney doesn't care they just want the work completed by a certain date. I'm a former cast member of the Magic Kingdom and I know people die there all the time. Heartbreaking.
