Rep. Hanabusa and I have formally requested a Congressional investigation into Saturday's false missile alert. The chain of failures must be immediately corrected, and those responsible must be held accountable so that this never happens again in Hawaiʻi or anywhere else.

An investigation is warranted because of the similar false alarm in Japan and because the password to the Hawaii system was accidentally leaked in a photo that included a Post-It note in the background with login credentials. It's possible the same person had a hand in both.
