Attachments can survive a long time after their (psychological) reason for existing is gone. They die when the reinforcement stops activating.

sometimes they die when 'reinforcement stops activating', but sometimes they need much more. Some subtle knots that have been driven into the body - knots in places you didn't know existed - take much more digesting. It is an excellent training, but requires excellent teacher/s.

That's a great point. I've experienced that rather directly, too - my meditation teacher frequently posts some insight on Twitter and I go "Oh," and suddenly I have it, too.
