Well, in the Czech Republic I had to pay a private doctor to get a sick note without having my appendix removed first.
I had mononucleosis.

Patient law is rly bad but partly reformed. Plenty of doctors and nurses remember a time of virtually no patient autonomy & act accordingly.

A bit of both. My regular doctor treated me like a human being, but her substitute while she was on holiday was an older ex-communist type.

But I was at an upper-end clinic that happened to also run with public health insurance. Scorn for patients was the general norm elsewhere.
Another time a doctor told me off for 'being out irresponsibly at an unreasonable hour,' when I'd been assaulted.
Got treatment, though.

Presumptuous doctor syndrome. We get a little of that too. You have to be a bit of a narcissist to be a doctor in the first place, I reckon.

For me as well! In Norway, the health care was delivered with considerably more respect. But they're merging hospitals like mad now; queues.
