      The Healthcare EO is out... https://www.whitehouse.gov/the-press-office/2017/10/12/presidential-executive-order-promoting-healthcare-choice-and-competition … #mepolitics #Trumpcare

Let me start by saying I am no fan of #ACA. I think it is mediocre law at best. I do think healthcare reform is badly needed. #mepolitics

BUT, if you are going to change law, make it better. Do *not* put transitional shortcomings on those least equipped to handle. #mepolitics

The EO starts out sounding nice. Problems in the 1st paragraph. There is this myth about buying insurance across state lines. #mepoliticspic.twitter.com/TX8OeCOHMB

I've had a lot of plans used over state lines. They were usable, but sometimes providers were "out of network". #mepolitics

Because of course they were. W/o single payer, it's impractical for insurers to negotiate with service providers states away. #mepolitics

Also across state lines, different policies/rules. #Maine has http://www.maine.gov/pfr/insurance/  #mepolitics

Not inherently good/bad, but adds complications/costs. It really isn't clear there would be any competition gains for consumers. #mepolitics

Also unclear that insurers have any desire to participate cross-state this way; my out-of-state claims seemed to annoy insurers #mepolitics

There have been premium increases & challenges keeping market places competitive, in part because they are not profitable. #mepolitics

Lack of profitability is driven by administrative efficiency (how well company works), risk pool (how many people/how diverse), #mepolitics

This EO's solution is AHPs, STLDI, and HRAs. I'm not a health insurance expert, so I'll do my best with these. #mepolitics

AHPs seem to be good & bad. They can provide self-employed/small employers (huge part of Maine) access in different ways. #mepolitics

This would provide those folks access to potentially less expensive insurance, but make the risk pools they leave less stable. #mepolitics

This creates different winners/losers, but tough to tell if there is a net gain. There would probably be a short term gain to #mepolitics

those who move, a short term loss to insurers. Everything would re-balance a year or so out, more $ pp into exchange...somehow. #mepolitics

A better choice might be to open up MaineCare to businesses/individuals to buy in. That would expand the state's risk pool. #mepolitics

Shoring up the risk pool, we could expand MaineCare access & provider compensation *without* a tax increase. By consumer choice. #mepoliticspic.twitter.com/2Dw0tG7hah

STLDI plans are generally worse than current ACA plans, though they may be less expensive...because they cover less. #mepolitics

Not the best analogy, these remind me of when you have a car & can't afford insurance so the lender is like "yo, we have this insurance"
