My father is utterly incapable of understanding British accents. I once accompanied him to meet with a British distributor for our family business where I acted as an English-English interpreter. The distributor was so tremendously confused. He thought it was a joke... at first.

Tbf when I lived in Indiana I had a brutish friend who would regularly meet with open looks of like horror and confusion bc people couldn’t understand him.

As silly as that sounds, I can see it. Because it isn't just the accent. It's cadence, pronunciation, pluralization of words, even sentence structure in some cases. I could see if you weren't exposed to it how it could be hard to understand. Like Indian tech support in english.
